When you arrive for your free consultation, you will need to complete paperwork before your appointment can begin.  Completing this ahead of time ensures that you are provided with all the time you need to have your questions answered. You can download the paperwork directly from this page or request that we send you it to you in the mail.  Remember to bring it with you to your consult appointment.

If you would rather complete the paperwork in our clinic at the time of your appointment, please arrive thirty minutes prior to your appointment time.
